Blue Cross sells Medicare Advantage in 58 of Oklahoma’s 77 counties for 2026 – BlueLincs HMO plans, a Dual Care Plus special needs plan, and a family of PPOs, several at $0 premium. Premiums as filed for 2026. Out-of-pocket maximums on these plans run from roughly $4,200 on Basic up to the mid-$7,000s on some $0 PPOs, and most $0-premium plans carry the full $615 Part D deductible. Exact figures vary by plan and county – we confirm against the official Summary of Benefits when we quote you. Some plans include a Part B premium giveback; the amount depends on the plan and county.
The Honest Review
1. The Saint Francis problem (Tulsa, read this first)
Since January 1, 2025, Warren Clinic primary care – the physician arm of Saint Francis, Tulsa’s largest health system – does not accept Medicare Advantage plans where it is out of network, and Blue Cross is on that list. Saint Francis currently states that CommunityCare and UnitedHealthcare are the only two Advantage plans with in-network access to Warren Clinic providers and Saint Francis facilities. Traditional Medicare is accepted system-wide – which means a BCBSOK Medicare Supplement keeps full Saint Francis access while BCBSOK Medicare Advantage does not. If your doctors are at Warren Clinic, this one fact should drive your whole decision.
2. Middling star ratings
The 2026 BCBSOK Medicare Advantage plans carry 3.0 to 3.5 star ratings – not bad, not great. The two standalone Blue Cross MedicareRx drug plans sit at 2.0 stars by BCBSOK’s own disclosure, which is genuinely poor; if you want a standalone Part D plan alongside a Medigap policy, we will usually shop it elsewhere.
3. Prior authorization is the Medicare Advantage tax
This is an industry problem, not a Blue Cross-specific one, but it is real: Medicare Advantage insurers made about 53 million prior authorization determinations in 2024, denied 7.7% initially, and among the small share of denials that were appealed, over 80% were overturned. Original Medicare plus a Supplement has almost none of this. Rural Oklahoma adds a wrinkle: Stillwater Medical ended all in-network Medicare Advantage contracts back in 2023 citing exactly this – verify any hospital you rely on before enrolling.
4. The extras are real, but capped
Dental, vision, hearing, OTC allowances, and SilverSneakers are genuine value on paper. Read the ceilings: hearing aid copays on the PPOs run several hundred dollars per aid, and dental annual maximums are modest. The extras are a nice bonus – they are not a reason to give up provider freedom you actually need.
What These Plans Genuinely Do Well
- $0 premiums in both HMO and PPO form across most of the 58-county footprint, with drug coverage built in on nearly every plan.
- Basic HMO is a legitimately strong value design – $0 premium with the lowest out-of-pocket ceiling in the lineup.
- Dual Care Plus is one of the few Oklahoma D-SNPs with meaningful extras for people on both Medicare and SoonerCare.
- Balance and Optimum give heavier utilizers a real lever – paying a premium to cut the out-of-pocket maximum and drug deductible.
Advantage or Supplement? The 60-Second Version
Medicare Advantage tends to fit if you:
- Are premium-sensitive and relatively healthy
- Are comfortable inside a network with referrals and prior auth
- Want dental/vision/hearing extras bundled in
- Have verified your doctors and hospital are in-network
A Supplement tends to fit if you:
- Want any Medicare provider in the country, no networks, no referrals
- Use Saint Francis / Warren Clinic, or travel a lot
- Value Oklahoma’s Birthday Rule – yearly carrier switching with no health questions
- Would rather pay a known premium than unknown copays

Does Saint Francis take Blue Cross Medicare Advantage?
Warren Clinic primary care does not accept out-of-network Medicare Advantage plans, including Blue Cross, and Saint Francis names CommunityCare and UnitedHealthcare as the only Advantage plans with in-network access. Traditional Medicare – including with a BCBSOK Medicare Supplement – is accepted system-wide.
What is BlueLincs HMO?
BlueLincs is the HMO network behind BCBSOK’s Medicare Advantage HMO plans, offered in the OKC and Tulsa metros plus select other counties. It is a Medicare network – do not use its directory to check an ACA or employer plan, and vice versa.
Are the $0 premium plans really free?
The premium is genuinely $0, but you still pay your Part B premium, plan copays and coinsurance up to an annual out-of-pocket maximum that runs several thousand dollars, and most $0 plans carry the full $615 drug deductible. “Free” plans cost money when you use them – that is the trade.
Can I switch from Medicare Advantage to a Supplement later?
You can leave MA during the annual enrollment windows, but joining a Supplement after your original Medigap open enrollment usually means health underwriting – approval is not guaranteed. This asymmetry is why we push people to make the Advantage-vs-Supplement decision carefully the first time. Note Oklahoma’s Birthday Rule helps Medigap holders switch between Supplements, not from Advantage into Medigap.
