CommunityCare Medicare Advantage in Oklahoma
CommunityCare Senior Health Plan is the Medicare Advantage arm of Oklahoma's only locally owned and operated health plan. It has served Oklahoma Medicare beneficiaries for more than three decades, and it is owned by the same two health systems that anchor its network - Saint Francis Health System and Ascension St. John.
Why local ownership matters on a Medicare plan
Most Medicare Advantage plans sold in Oklahoma are run from out of state. CommunityCare is not. Its service area, provider contracts, and member services are all built around northeastern Oklahoma, and the plan and the hospitals share ownership. In practice that tends to mean fewer surprises on whether a Saint Francis or Ascension St. John facility is in network, because the answer is not being negotiated between two unrelated companies.
The tradeoff is geographic. A national carrier will follow you across state lines more easily. If you spend several months a year outside Oklahoma, that is worth raising before you enroll.
What to look at before you choose
- Your doctors. CommunityCare is an HMO. You generally need to stay in network and use a primary care physician, so confirm your specific doctors and hospital before you switch.
- Your prescriptions. Effective January 1, 2026, CommunityCare moved to Navitus Health Solutions as its pharmacy benefit manager. If you were already a member, your pharmacy network and drug pricing may not behave exactly as they did in 2025. Re-check your drug list.
- Your county. CommunityCare files a county-level service area concentrated in and around Tulsa. It is not available statewide.
- Star rating. CMS rates plans annually. Ratings change every year, so use the current year rating rather than what you remember from last time.
How CommunityCare compares in Oklahoma
CommunityCare competes with national Medicare Advantage carriers across Oklahoma. Where it tends to win is coordination inside the Saint Francis and Ascension St. John systems. Where a national plan may fit better is travel, a broader multi-state network, or a county CommunityCare does not serve. There is no single best plan - there is the plan that covers your doctors and your drugs at the lowest total cost.

Does CommunityCare offer a Medicare Supplement instead?
CommunityCare Senior Health Plan is a Medicare Advantage plan, not a Medigap policy. If you want Original Medicare plus a Medicare Supplement, we can compare that route alongside it.
What changed for 2026?
The most significant change is the pharmacy benefit manager moving to Navitus Health Solutions effective January 1, 2026. Benefits and networks are refiled with CMS annually, so review your Annual Notice of Change.
